Severe decay. When a cavity has progressed so far that insufficient healthy tooth structure remains for a crown or filling to restore it, extraction removes the source of infection and pain. The earlier a significant cavity is caught at a routine cleaning, the more options exist for saving the tooth.
Cracked or fractured tooth. A crack extending below the gumline or splitting the root cannot be repaired. Extraction followed by bone grafting and eventual restoration is often the cleanest path forward.
Advanced gum disease. When periodontal disease has caused significant bone loss around a tooth, it may become too loose to function. Extraction prevents the infection from spreading further.
Impacted wisdom teeth. Wisdom teeth that are impacted, partially erupted, or positioned to threaten adjacent healthy teeth often need removal. We handle routine wisdom tooth extractions in-house at our Dallas location — more complex surgical impactions are coordinated within our Thrive network.

Nitrous oxide. Nitrous oxide — laughing gas — is available at our Dallas office for any patient who wants additional comfort during their extraction. Inhaled through a small comfortable mask, it creates a feeling of calm and relaxation within minutes. The local anesthetic handles all pain prevention — the nitrous handles the anxiety. Nitrous clears completely within minutes of removing the mask so most patients drive home and return to normal activity the same day.
Two TVs and Netflix in your treatment room. Every operatory at our Dallas location has two TVs — one above the chair and one at eye level — with Netflix throughout your appointment. Comfort headphones in every room. A pillow and blanket if you want them. A warm towel at the end. Your X-rays on screen before we begin. Digital X-rays and intraoral photos are displayed on the treatment room TVs before any treatment discussion begins. Simple extractions involve fully erupted teeth that are structurally intact above the gumline. The tooth is loosened with a dental elevator and removed with forceps. Straightforward, quick, and handled routinely at our Dallas location under local anesthetic with optional nitrous oxide.
Surgical extractions involve teeth broken at or below the gumline, impacted below the surface, or positioned in a way that requires removing some surrounding tissue or bone for access. More involved but still performed in-office at our Dallas location for most cases.
For cases more complex than what should be handled in-house, we coordinate within our Thrive network first — your records travel with you seamlessly. Truly complex cases go to trusted outside oral surgeons we work with directly. You’ll always know before your appointment whether we’re handling it in-house or coordinating a referral.
The first 24 hours — the most critical period:
Bite firmly on gauze for 30–45 minutes to help the blood clot form in the socket. The clot is essential — it protects the underlying bone while healing begins. Strictly avoid anything that creates suction or pressure: no straws, no smoking or vaping, no forceful rinsing or spitting, no carbonated beverages. These can dislodge the clot and cause dry socket — the most common and most painful post-extraction complication. Apply a cold compress to the outside of your cheek in 15–20 minute intervals to reduce swelling. Stick to soft foods.
Days 2–4:
Continue soft foods. No straws, no smoking or vaping. Begin gentle warm salt water rinses after meals — let the liquid rest over the site and drain out rather than swishing.
Prescriptions:
If pain medication or antibiotics are needed, our Dallas team prescribes at your appointment. The nearest pharmacy is just minutes from our Frankford and Preston Road office — your prescription is filled quickly. No extra trip across Dallas while you’re recovering.
When to call us:
Pain that intensifies after day three rather than gradually improving, significant swelling, fever, or a persistent unpleasant taste — call us promptly. Both dry socket and infection are treated quickly when addressed early.
Extraction is the beginning of the next chapter — not the end of the story. Once the socket heals — typically four to six weeks with bone grafting — we discuss your options for restoring the gap.
Dental bridge: A fixed, non-removable restoration anchored to adjacent teeth. A strong, long-lasting option when neighboring teeth are healthy and bone grafting was performed. We use the Medit scanner for all bridge fabrication — no impressions — and place same-day temporaries so you’re never without a restoration during fabrication.
Partial denture: A removable appliance replacing one or more missing teeth. More affordable upfront and practical when adjacent teeth aren’t strong enough for a bridge or when multiple teeth in different areas are missing.
Full dentures: For patients missing most or all teeth. We offer full, partial, and immediate dentures at our Dallas location. Immediate dentures are placed the same day as extractions so patients never go without teeth during healing.
We discuss all replacement options at your extraction appointment. No patient leaves our Dallas office after an extraction without a clear picture of what comes next — timeline, cost, and what each option involves.
Dry socket is the most common complication after extraction and occurs when the blood clot dissolves or is dislodged before healing is complete — exposing the underlying bone. It is intensely painful and develops two to four days after extraction when pain that was improving suddenly worsens significantly.
Prevention is almost entirely within your control for the first 72 hours: no straws, no smoking or vaping, no forceful rinsing or spitting, no carbonated beverages. Following these instructions carefully is the single most important thing you do after an extraction.
If dry socket develops, call us immediately. We treat it promptly with a medicated dressing that relieves pain quickly while healing resumes. Most patients feel significantly better within 24 hours of treatment.

The extraction itself is performed under local anesthetic — you feel pressure and movement but should not feel pain. Nitrous oxide is available at our Dallas office for additional comfort support. After the anesthetic wears off, mild to moderate soreness for two to three days is normal and manageable with over-the-counter pain medication. Most patients tell us it was significantly more manageable than expected.
A simple extraction typically takes 20–30 minutes from anesthetic to completion. Surgical extractions take longer depending on complexity. Bone grafting adds a few minutes. Plan for a total visit of about an hour including check-in, anesthetic, procedure, and post-procedure instructions.

Without it, the bone resorbs naturally over the following weeks and months — limiting future restoration options.
